Best Probiotic Strains Guide: The Right Strain for Every Condition

Probiotics are strain-specific -- a strain that works for IBS may do nothing for anxiety, and vice versa. Understanding which strains have evidence for which conditions transforms probiotic use from guesswork into targeted therapy.

Why strain specificity matters

Probiotic species (like Lactobacillus acidophilus) contain thousands of strains, and different strains of the same species can have dramatically different effects. The clinical evidence for probiotics is strain-specific -- a study showing Lactobacillus rhamnosus GG prevents antibiotic diarrhoea tells you nothing about other Lactobacillus rhamnosus strains. This is why generic "multi-billion CFU probiotic" marketing is misleading -- what matters is whether the specific strains in the product have clinical evidence for your condition. Always look for strain identity on the label (Lactobacillus rhamnosus GG, for example, with the species and strain designation).

For IBS and digestive symptoms

Bifidobacterium infantis 35624 (Align): most consistent IBS evidence, significantly reduces abdominal pain, bloating and bowel urgency. Lactobacillus acidophilus NCFM + Bifidobacterium lactis Bi-07: RCT evidence for IBS symptom reduction. VSL#3 (high-dose multi-strain mixture): evidence for IBS-D and ulcerative colitis. Saccharomyces boulardii CNCM I-745: reduces IBS-D frequency and traveller's diarrhoea. Lactobacillus plantarum 299v: reduces IBS bloating and abdominal pain.

For antibiotic-associated diarrhoea and C. difficile prevention

Lactobacillus rhamnosus GG (Culturelle): the most studied probiotic for antibiotic-associated diarrhoea -- take during and for 2 weeks after antibiotic course. Saccharomyces boulardii CNCM I-745: significantly reduces both antibiotic-associated diarrhoea and C. difficile infection prevention. These should be taken 2 hours apart from the antibiotic to prevent the antibiotic killing the probiotic before it reaches the gut.

For mental health and mood

Lactobacillus helveticus R0052 + Bifidobacterium longum R0175 (the "psychobiotic" combination, sold as Probio'Stick): demonstrated antidepressant and anxiolytic effects in multiple RCTs. Lactobacillus rhamnosus JB-1: reduced anxiety and altered GABA receptor expression in animal studies; promising early human trials. Bifidobacterium longum NCC3001: reduced depression scores in IBS patients in a pilot RCT.

For immunity and respiratory infections

Lactobacillus rhamnosus GG: reduces incidence and duration of respiratory infections in children and adults. Lactobacillus acidophilus NCFM: reduces cold severity and duration. Bifidobacterium animalis subsp. lactis BB-12: reduces respiratory infection incidence. Multi-strain products containing Lactobacillus and Bifidobacterium species generally show consistent immune benefits.

For women's health

Lactobacillus rhamnosus GR-1 + Lactobacillus reuteri RC-14 (Fem-Dophilus/RepHresh Pro-B): specifically colonises vaginal mucosa, restores healthy vaginal flora, reduces bacterial vaginosis and yeast infection recurrence. This is the best-evidenced probiotic for female urogenital health and requires vaginal-specific strains not found in generic gut probiotics.

Frequently Asked Questions

Which probiotic is best for IBS?

Bifidobacterium infantis 35624 (sold as Align) has the most consistent IBS-specific evidence from multiple RCTs. Lactobacillus plantarum 299v and VSL#3 also have good evidence. The key is using a strain with documented IBS evidence rather than a generic multi-billion CFU product with unknown strain identity.

Should I take probiotics during antibiotics?

Yes -- take a probiotic containing Lactobacillus rhamnosus GG or Saccharomyces boulardii CNCM I-745 during antibiotic treatment and for 2 weeks after. Take the probiotic 2 hours apart from the antibiotic dose. This reduces antibiotic-associated diarrhoea risk by 30-50% and significantly reduces C. difficile infection risk.

Do probiotics help with anxiety and depression?

The combination of Lactobacillus helveticus R0052 + Bifidobacterium longum R0175 has demonstrated anxiolytic and antidepressant effects in multiple RCTs -- the most evidence for mental health of any probiotic combination. This gut-brain axis modulation works through multiple mechanisms: reducing gut permeability (which drives systemic inflammation affecting the brain), modulating the vagus nerve, and producing GABA and serotonin precursors.
