Natural Remedies for Cough: What Actually Works

Cough is the most common reason for GP consultation. Most acute coughs are post-infectious and self-limiting; several natural interventions have good evidence for reducing duration and severity.

Honey -- the most evidence-backed natural cough remedy

Honey is the most evidence-backed natural cough treatment, particularly in children. A 2012 Cochrane-referenced meta-analysis found honey significantly superior to placebo and over-the-counter cough suppressants (dextromethorphan, diphenhydramine) for reducing cough frequency and severity in children, and improving sleep quality. A 2021 BMJ Evidence Review confirmed honey's superiority to usual care for acute upper respiratory infections including cough. Mechanism: honey coats the pharynx (demulcent effect reducing cough reflex stimulation), has direct antimicrobial properties, and anti-inflammatory activity through polyphenol content. Dose: 2.5-10ml (one to two teaspoons) at bedtime. Not safe for children under 1 year (botulism risk).

Thyme (Thymus vulgaris)

Thyme has documented bronchodilatory (relaxing airway smooth muscle) and expectorant (promoting mucus clearance) activity through its volatile oils (thymol and carvacrol). Multiple German RCTs find thyme extract (liquid extract or syrup) significantly superior to placebo and comparable to ambroxol (a standard expectorant) for productive cough with bronchitis. A 2010 trial found thyme-ivy syrup superior to ambroxol for acute bronchitis. Particularly effective for productive (wet) coughs with bronchitis. Available as thyme syrup, thyme honey, or infusion of fresh thyme in hot water.

Ivy leaf extract (Hedera helix)

Ivy leaf extract is one of the most evidence-backed herbal cough remedies in Europe -- widely used in German paediatric practice. Its saponins have expectorant activity (thinning and mobilising mucus) and mild bronchodilatory effects. Multiple RCTs including a large comparative trial confirm ivy leaf extract significantly reduces acute bronchitis symptoms compared to placebo and comparably to pharmaceutical expectorants. Available as Prospan syrup (the most studied commercial preparation) and other ivy leaf standardised extracts.

Positioning and steam

For nocturnal cough: elevating the head of the bed (15cm) reduces post-nasal drip that triggers cough reflex at night. Sleeping on the side (rather than back) reduces post-nasal drip pooling in the laryngopharynx. Steam inhalation (with optional eucalyptus) reduces mucosal dryness and thins secretions, reducing cough trigger stimulation. For productive cough, adequate hydration (2+ litres daily) maintains mucociliary clearance and keeps secretions mobile.
