Natural Remedies for PCOS: What the Clinical Evidence Shows

PCOS affects up to 10% of women of reproductive age and is the most common cause of female infertility. Its core mechanisms -- insulin resistance, androgen excess, chronic inflammation and ovulatory dysfunction -- respond meaningfully to targeted natural interventions.

Understanding the root cause

PCOS is primarily an insulin resistance problem. Elevated insulin drives the ovary to produce excess androgens (testosterone, DHEAS), which disrupt follicle development and prevent ovulation. Elevated androgens drive the characteristic symptoms: acne, hirsutism, hair thinning and irregular cycles. The oral contraceptive pill suppresses symptoms by shutting down ovarian function but does not address underlying insulin resistance -- symptoms return on stopping. Natural interventions that address insulin resistance can produce genuine remission.

Myo-inositol -- the most evidence-backed intervention

Myo-inositol (a B-vitamin precursor) is a second messenger in insulin signalling. PCOS ovarian cells are selectively deficient in D-chiro-inositol, impairing the insulin signalling needed for normal follicle development. Supplementing myo-inositol and DCI at a 40:1 ratio restores this signalling. Multiple RCTs demonstrate myo-inositol (2-4g daily) significantly improves insulin sensitivity, reduces testosterone, restores regular ovulation, improves oocyte quality and increases live birth rates in IVF. A 2019 meta-analysis found inositol comparable to metformin with dramatically fewer side effects.

Spearmint tea for androgen reduction

Drinking 2 cups of spearmint tea daily for 30 days significantly reduced free and total testosterone in PCOS women compared to chamomile control in a published RCT. A follow-up pilot RCT confirmed the finding. One of the most accessible and safest natural interventions for PCOS androgen excess.

Low-glycaemic diet

A low-GI diet minimising blood sugar and insulin spikes -- legumes, vegetables, wholegrains, berries over refined carbohydrates -- has the strongest dietary evidence for PCOS. A 2021 meta-analysis found it significantly reduced insulin resistance, testosterone and menstrual irregularity. Even 5-10% weight loss in overweight PCOS women can restore ovulation through reduction in peripheral androgen production.

Berberine

Berberine (from barberries, goldenseal) is a potent natural insulin sensitiser. Multiple RCTs find berberine (1,500mg daily in divided doses) comparable to metformin for improving insulin resistance, reducing testosterone and restoring ovulation in PCOS women. It activates AMPK (the enzyme metformin targets) and additionally modulates gut microbiome composition. Take with meals. Not appropriate during pregnancy.
